Registration: Any resident child who will be five years of age on or before December 1st of that school year is eligible for admission to Kindergarten. Registration for Kindergarten is held in the winter of the year prior to entrance. Documentary proof of the child’s date of birth must be presented at the time of registration. Health immunization information must be documented
. Parents of children not registered in the winter should contact their neighborhood school or contact the District Office. Registration of new elementary pupils (other than Kindergarten) is handled by the principal. Parents of new pupils are urged to contact the principal in advance by phone. Secondary school registration is arranged through the guidance department. A contact by phone will facilitate the registration. Appo
intments should be made since parents must accompany the new pupil. Information required at registration includes birth certificate, health records, previous school grades (report cards), and proof of residency in the district. If there is any doubt as to the school your child should attend, telephone the District Office.
Immunization: All students must meet the immunization requirements as set forth in Section 2164 of the New York Public Health Law. The following immunizations are required and the school nurse must have the proper documentation or a student is subject to being excluded from school:
- three or more doses of diphtheria toxoid.
- three or more doses of oral polio or four of more doses of inactivated poliomyelitis vaccine (IPV) administered after 1968.
- one dose of live measles virus vaccine if born before 1985. Two doses if born on or after January 1, 1985, or physician documentation of positive measles diagnosis.
- a single dose of live rubella virus vaccine administered after the age of 12 months or serologic evidence of immunity.
- a single dose of live mumps virus vaccine administered after the age of 12 months, or physician diagnosed mumps disease, or serologic evidence of immunity.
- Hepatitis-B, three doses if born on or after January 1, 1993. Effective September 1, 2000, all children entering the 7th grade must have received three doses of Hepatitis-B.
Working Papers: The middle and high school Guidance Departments provide applications for working papers. This application requires certification of health and a parent’s signature. The working card must be signed in the presence of a staff member.
Student Accident Insurance: The Board of Education provides schooltime non-duplicating accident insurance for all students in kindergarten through grade 12. In addition, all injuries resulting from interscholastic athletics, including football, are also covered by the insurance plan.
The accident insurance coverage pays for injuries sustained while in attendance of classes, while traveling directly or without interruption to or from the insured’s residence and school for regular school sessions; and while participating in or attending school functions and supervised activities including all sports.
Students who are injured during regular school hours or during an after-school activity, including interscholastic athletics, are to report their injury to their teacher or coach, and to the school nurse assigned to their school.
The plan will help pay the cost of treatment for accidental bodily injury up to the specified medical limit per claim. Since the school’s student accident insurance plan is non-duplicating, the individual’s coverage is limited to hospital and surgical costs that were not covered under any group or blanket insurance the family might have.
Health Examinations: Opportunity for FREE health examinations for working papers and participation in sports will be provided in the schools as in the past. However, students who choose to use their family physician at their own expense, MUST see the school nurse for the proper forms and return these forms to the nurse for final approval by the school ph
ysician.
The State requires that each child in grades Kindergarten, 1, 3, 7 and 10 receive a physical examination. The District will schedule these to be done in school if the parents choose to have it done by our school physician. However, usually it would be advisable for the examination to be done privately by the physician who ordinarily cares for the child, since that physician will be more
familiar with the child and his/her medical history.
Student Medication: Medication must be delivered to the school nurse by the parent or guardian, in the original container.
Parents are requested to cooperate with the school nurse in completing the requirements of the law. Exceptions allowed by the law will be honored; however, parents are reminded that the law requires that students not meeting all requirements must be excluded from school. |
