Immunizations & Physicals
In an effort to streamline the requests for medical documentation, please see the required medical documentation breakdown by grade.
NYSED requires a physical exam for new entrants and students in Grades Pre-K, K, 1, 3, 5, 7, 9 & 11; annually for interscholastic sports; and working papers as needed; or as required by the Committee on Special Education (CSE) or Committee on Preschool Special Education (CPSE).
If you need assistance in obtaining a physical examination or immunization, please contact your school nurse, who can share resources with you.
Please use this physical exam form when taking your student for their annual physical.
