To administer any medication—whether prescription or over-the-counter—we must have:

1. A written order from a licensed healthcare provider

2. Written permission from a parent or guardian

Please note:

• Medications must be delivered by a parent or guardian in the original, labeled container.

• All medications will be stored securely in the nurse’s office.

• Students are not permitted to bring medication to school on the bus, as this is prohibited by New York State law.

Physicals

NYS law requires students in grades K, 1, 3, 5, 7, 9, and 11, as well as new entrants to the district, to have a physical examination. Please forward a copy to our office for us to keep on file. Immunizations

Students must have an up-to-date immunization record, which complies with New York State Department of Health requirements. If you have any questions regarding required vaccinations, you can contact us here in the nurse's office, call your healthcare provider or refer to the NYS Department of Health chart of required vaccinations for school attendance by clicking the tab at the left.
