All 3rd and 5th graders are screened for far vision and hearing.
New York State requires a Physical completed by a health care provider for all 3rd and 5th graders. (Physicals are now required for grades K, 1, 3, 5, 7,9, and 11.) This should include the most recent copy of immunizations.
If your child needs to take any medicine at school, your health care provider must complete a “medication at school” form and you must sign it as well. This form needs to be completed every year. I cannot give any medications without a signed form!
All medications must be brought to school by an adult. It is against NYS law to have children carry medication on the school bus.
A fever is 100.4 or above. Your child should not come to school with any temperature of 100.4 or over. We want your children in school but not if they are sick!
The following is very important: Make sure you have given us several phone numbers to reach you in case your child gets sick or is injured. It is essential that we be able to contact you at all times. Make sure you have filled out the emergency information completely and update us during the year of any phone number changes.
I am at school from 8:30 am to 3:30 pm each day. You are welcome to call me or stop by at any time. I check my email frequently during the day. The more I know about your child, the better I can care for them.

When to Keep Your Child Home from School:
Please use the following guidelines to help stop the spread of illnesses in our school. Your children’s health is foremost on our minds.
- Please keep your child home from school if he/she has a fever (often a temperature of 100.4 or higher signals an illness or contagion). Your child must be fever free for a full 24 hours before returning to school, without the use of a fever reducing medication, such as Tylenol, Motrin or Advil.
- Please keep your child home if he/she vomited or had diarrhea the night before or the morning of school. Please maintain the 24 hour rule; vomit free and/or diarrhea free; as well as being able to tolerate a regular diet for 24 hours before the child can return to school.
- Please give your child a full 24 hours at home, once placed on antibiotics for contagious diseases such as: strep throat, bronchitis, conjunctivitis (pink eye) and impetigo.
- Please keep your child home if they exhibit flu-like symptoms. Contact your child’s provider for: fever accompanied with chills, cough, sore throat, runny nose, muscle pain, fatigue, persistent vomiting and diarrhea.
Additionally, please keep your child home if:
- His/her cough is not under control. (Please note; cough drops, and over-the-counter cough medicine, require a doctor’s note).
- He/she has a rash accompanied by a fever or behavioral changes. It is recommended the child be seen by their pediatrician.

Medications in School:
Medication administration in school requires a written order from a licensed prescriber along with written parental permission. All medications (prescription and over-the-counter) must adhere to these guidelines. Medication must be brought in by a parent/guardian (adult) in the original container. Physicals:
NYS law requires students in grades K, 1, 3, 5, 7, 9, and 11, as well as new entrants, to have a physical examination. NYS law also requires Body Mass Index (BMI) and weight-status-category for students in grades K, 1, 3, 5, 7, 9, 11, as well as new entrants, to be reported for these grade levels.
Gym and recess: if a student has been instructed to not participate in Physical education (gym class) they will not be allowed to participate in Recess. A note from a parent will only excuse a child for one day of gym.
