2024-2025 NYS BMI School Survey
Brewster Central School District has been selected to take part in a BMI survey by the NYS Department of Health. As part of the survey, we will be reporting to NYS Department of Health information about our students' weight status groups. ONLY SUMMARY INFORMATION is sent. NO NAMES and no information about individual students is sent. However, you may choose to have your child's information excluded from the survey report. Please complete the attached opt-out form and give it to your child's school nurse.

Immunizations, Physicals, and Medications
In an effort to streamline the requests for medical documentation please see the required medical documentation breakdown by grade. NYSED requires a physical exam for new entrants and students in Grades Pre-K, K, 1, 3, 5, 7, 9 & 11; annually for interscholastic sports; and working papers as needed; or as required by the Committee on Special Education (CSE) or Committee on Preschool Special Education (CPSE).

Universal Pre-K:
- Physical within the past 12 months
- 4 doses of DTaP (Diphtheria and Tetanus toxoid-containing vaccine and Pertussis vaccine), 3 doses of IPV/OPV (Polio vaccine), 1 MMR, 3 doses of Hepatitis B and 1 dose of Varicella (Chickenpox vaccine) 1–4 doses Haemophilus influenzae type b conjugate vaccine (Hib), 1 to 4 doses Pneumococcal Conjugate vaccine (PCV)
Kindergarten:
- Physical within the past 12 months
- -4 to 5 doses of DTaP (Diphtheria and Tetanus toxoid-containing vaccine and Pertussis vaccine), 3 to 4 doses of IPV/OPV (Polio vaccine), 2 MMR, 3 doses of Hepatitis B and 2 doses of Varicella (Chickenpox vaccine).
1st Grade:
- Physical within the past 12 months
2nd Grade:
- Physical may be requested for CSE
3rd grade:
- Physical within the past 12 months
4th Grade:
- Physical may be requested for CSE
5th Grade:
- Physical within the past 12 months

6th Grade:
- 1 dose of Tdap (Tetanus and Diphtheria toxoid-containing vaccine and Pertussis vaccine booster)
- Physical may be requested for CSE
7th Grade:
- Physical within the past 12 months
- 1 dose of (MenACWY) Meningococcal conjugate vaccine
8th grade:
- Physical within the past 12 months required for CSE, sports participation, and/or working papers (age 14+)
9th Grade:
- Physical within the past 12 months
10th Grade:
- Physical within the past 12 months required for CSE, sports participation, and/or working papers.
11th Grade:
- Physical within the past 12 months
12th Grade:
- Physical within the past 12 months required for sports participation
- 2 doses or 1 dose of (MenACWY) Meningococcal conjugate vaccine (if the dose was received at 16 years of age or older)

If you need assistance in obtaining a physical examination or immunization please contact your school nurse who can share resources with you.
Additional information
- A physical examination completed at your pediatrician meets the requirements for BCSD sports participation
- Physicals are valid for 12 months from the date of the exam
- Any medication orders for any medication (OTC or RX) to be administered at school need to be renewed annually. This includes any “self-carry” medication
- Use of any orthopedic devices at school (crutches, CAM boot, cane, etc) requires a note from a doctor with the anticipated duration of need.
Medication in school
All medications (prescription and over-the-counter) must adhere to these guidelines:
- Any medication orders for any medication (OTC or RX) to be administered at school need to be renewed annually. This includes any “self-carry” medication
- Medication administration in school requires a written order from a licensed prescriber along with written parental permission.
- Medication must be brought in by a parent/guardian (adult) in the original container. Medications CAN NOT be brought to school by a student via the bus.
